Case Management Certificate Program

Curriculum

The case management curriculum will cover the following topics:

Session 1 - The State of the Profession
Session I (8 hours) provides an introduction to the UCF Case Management Continuing Education Program and examines the state of the case management profession. The session will focus on issues and trends, theoretical concepts, standards of practice, evolving case management models of delivery and public policy relative to professional practice. Emphasis is on case management in the Central Florida community.

Objectives: Following Session I, participants will be able to:
  1. Discuss the historical development of case management.
  2. Utilize theoretical concepts as a foundation for professional practice.
  3. Link professional practice to public policy.
  4. Identify the issues and trends in case management.
  5. Define standards of care, models and terms across disciplines and systems.
  6. Discuss case management in the Central Florida community.

Session 2 - Best Practices in Case Management
Session II (8 hours) provides an overview of best practices in case management, evidenced based practice and best practice benchmarks. This session provides the opportunity to increase leadership and case management skills through interaction with professional peers and guest speakers. This session concludes with the practical application of best practice concepts in a case study.

Objectives: Following Session II, participants will be able to:
  1. Define best practice concepts in case management.
  2. Discuss the case manager as an agent of change.
  3. Identify public and private resources for the "toolbox."
  4. Apply best practice concepts to assessment and plan of care.
  5. Develop a plan of care based on best practice concepts.

Session 3 - Best Practices Across Populations
Session III (8 hours) provides an opportunity to apply best practice concepts when an individual approach or a population approach is required for services. Emphasis will focus on the integration of services across disciplines, systems and the 211 program to provide a continuum of services, which result in effective, cost efficient outcomes. This session provides an opportunity for multiple disciplines to develop an understanding of community resources and the opportunity for collaboration. This session will focus on an interactive lecture and conclude with group activities and case study.

Objectives: Following Session III, participants will be able to:
  1. Discuss population based case management models.
  2. Define the components of the disease case management model.
  3. Determine the differences in behavioral health case management models.
  4. Identify community resources/tools available for specific populations.
  5. Relate best practice concepts to population based case management.

Session 4 – Integration of Care Management
Session IV (8 hours) provides an overview of care giving from the perspective of the caregiver, family and the case manager as an individual. This session serves to provide an understanding of the economics and theories that guide ethical decision making processes, teach the value of diversity, engage families, and resolve conflicts encountered in the integration of the care management process.

Objectives: Following Session IV, participants will be able to:
  1. Discuss the relationship between case management and care giving.
  2. Discuss ethics, personal beliefs and values relative to case management.
  3. Recognize the value of diversity in the central Florida community.
  4. Identify techniques for managing angry people and resolving conflicts.
  5. Practice interpersonal skills for engaging families in the case management process.
  6. Reinforce the role of the case manager as a change agent for care giving.

Session 5 – Professional Development
Session V (8 hours) presents the opportunity for professional development. The session will focus on professional organizations and opportunities for certification and continuing education and the projected expansion of case management as a profession. Advanced case management tools, time management, work management and technology will be presented. The session will be divided into a 6 hour interactive lecture and conclude with a 2 hour interactive panel discussion on networking, resources and professional relationships within the Central Florida community.

Objectives: Following Session V, participants will be able to:
  • Discuss the projected future of case management as a profession.
  • Discuss case management information systems and technology.
  • Identify professional organizations at national and local levels.
  • List professional certifications that may enhance professional advancement.
  • Develop a self-care plan to manage stress, avoid fatigue and burnout.
  • Utilize time and work management techniques to increase productivity.
  • Increase professional network through collaborative community relationships.
